Orphaned Kitten

I found a kitten out in the backyard last night. He'd been out by himself for a couple of hours and was crying pretty good. Because it was so windy and cooling down I brought him in. Anyone know how to mix kitten formula???? I used to have the recipe from my vet on how much condensed milk/whatever else. Unfortunately, I can't find it.

Kitten is 4 weeks old. I scolded the mama(she's a stray who has claimed me). She came up and got her little one and all is good.
