Personally, I pray to the Great Goddess, the Creatrix. :sunny:

Dragon-- pray to whoever you think will be the most helpful in healing/comforting/whatever your reason for prayer is. It's completely up to you. As for how you pray, again, your choice. You could kneel down, hands clasped, or drum a little, or dance around, or light candles, or just talk aloud to a deity, or sing... whatever you feel is best. :) There's no right answer.
Personally, I'm praying to Kali lately, because I've started to really form a connection to her and her myth, and it's what comforts me. :)

Hekate when I need strength.
Demeter or Gaia when I need comfort.
Sometimes, in times of great pain, I find myself praying to Jesus as Lord still. I used to get worried and feel guilty or ashamed to admit that, but why?
I still have issues with Yahweh and the religion of my youth at times, but Jesus' message is ok by me.

Who you pray to is up to you, but I think that it is your intention that is most important. Pray with love, honesty, and sincerity and I am sure you will be heard. I do not think you have to have a specific way of saying things.
Personally, I pray to God the Father, the Son, and the Holy Spirit and I pray to the guardian angels that I believe everyone has.

Everyone's advice so far has been very good. I feel that you should pray to whoever you feel most comfortable with. In the last few days I've found great comfort with Isis, Kuan Yin and Jesus.
Personally, I will often sit in front of my altar, light a candle, and focus on feeling the presence of the Divine with me, then I will just pray to whoever comes to mind and say what's on my mind. Sometimes I make up things, sometimes I read out prayers written by others or myself as affirmations of what I am feeling or wanting to achieve.
