I'm on my fourth page of a google search and nothing. Not even an anonymous credited. Grr. I'm adding it to my bos, and I'd really like to know who to credit this to. As well as being able to find what else they've written, since I like it so much.
So, if anyone recognizes it, please let me know!

The invocations are standards in the public domain. I don't know the author, but its one of those that's been circling the 'Net for years. The rite itself I've never seen before.
