Anything you feel is important, to your faith, to you, to your gods...
As well as all the 'usual' stuff - my favourite books pertaining to my faith, crystals etc - I keep my paintbrushes on mine because art and creativity is an important part of my life. There really is no 'should' or 'shouldnt' when it comes to altars imo :)

Even elemental items are not always present on an altar, it is strictly a matter of preference. If you work within the chinese system of elements you would therefore have nothing for Air, but instead additional items for wood and metal. If you work with feng shui elements then you would have a total of eight all together, and if you follow a more traditional celtic ideaology you would have three to four, depending on whether you wish to recognize Fire along with earth, sky and sea.
Regardless of that, what goes on an altar is not necessarily things which are important. I have many important things in my life, some of which I would never place on my altar. What would go on an altar is anything you deem to be greatly sacred and sacred is different from important. Outside of that which is sacred, that which you utilize may go there as well, which anything from this group can likewise be viewed as sacred, depending on your perspective. Things such as a wand, a chalice, a censer, candle holders (not every candle holder you own, but those you regularly use each time)... but you may prefer to store most tools away and out of sight.
That covers a basic altar.
